The Prescription Assistance Program is available as a free service to assist patients in completing applications for discounted or free prescription medications through the manufacturer.
Each manufacturer has its own eligibility criteria, requirements, and application form. This program exists to assist patients with the application process at no cost to the patient.

Each medication manufacturer has its own requirements, but most are based on items such as:
- Income
- Insurance status
- Age
- Disability
What is included:

The following are typically included in the program
- Insulin
- Blood pressure medication
- Inhalers
What is not included:

The following medications are not included in the Prescription Assistance Program
- Pain medications
- Anxiety medications
Approval Information:
The manufacturer will notify MOFMC about your eligibility and our office will call you when your medication is ready to pick up.
