Eligibility Criteria:

The patient must be a patient at MOFMC and be referred by their primary care provider. Explore behavioral health treatment options.

Insurance/Cost Information:

Evaluation and treatment will be billed to the patient’s insurance, but coverage is not guaranteed. Patients with no insurance can apply for MAP.

If you or someone you know has feelings of worthlessness, hopelessness, lack of interest in things, trouble concentrating, anxiety, appetite or sleep disturbance you may benefit from counseling. Thoughts of self-harm should be addressed immediately. Counseling can help with stress management, grief, relationships, or major life changes.
