Spanish translation services are free and available upon request.
On Saturdays and holidays we may not have a translator available.  Please bring someone over the age of 18 with you to translate on Saturdays and holidays.

Important Forms:

  • New Patient Information Packet: This packet should be completed and returned to our office for patients who would like for MOFMC to begin serving as their Primary Care Provider. This is for patients that are transferring their care from another clinic to MOFMC.
  • Walk-in Patient Information Packet: This is for patients who have primary care at another office and do not want to transfer their medical records and care to MOFMC, but only want to utilize the MOMFC Walk-in Clinic services or be seen in the Concussion Clinic.
  • Access our patient portal.

Steps to becoming an established patient:

  • Complete the following packet completely.
  • The records release page must be completed in the packet so we know where to request your previous medical records.  If there are no medical records for us to request, please indicate this on the correct page of the packet.
  • Once completed, you may drop off at the front desk, fax the forms to 919-658-5754, or email the forms to /new-patients.  
  • We will begin to process your information and request your medical records in the order the packets are received.
  • One of our staff members will call you once we have received your records to schedule an appointment.
